Firearms Application forms can be obtained through the Records Section of the Police Division.

The Records Section is open from 8:00 am to 4:00 pm, Monday through Friday (except holidays).

Most Firearms Application forms can also be accessed on-line 24 hours a day, seven days a week.

Below is a breakdown of the required items needed for Firearms Applicants. Click on the appropriate link to access the necessary form(s).

If you need an:

  • Initial New Jersey Firearms ID Card
  • Initial Application for Handgun

You will need to:

  • Complete form: NJSP S.T.S. 33
  • Complete form: NJSP SP-66
  • Submit $5.00 in Cash, Check or Money Order made payable to: Borough of Beachwood for the initial Firearms ID Card
  • Submit $2.00 in Cash, Check or Money Order made payable to: Borough of Beachwood for each handgun permit
  • Submit to fingerprinting at a local Morpho-Trak facility.

    • The fee for fingerprinting is currently $57.20, payable directly to Morpho-Trust

NOTE: YOU WILL ALSO NEED TO CONTACT DIANE IN OUR RECORDS SECTION FOR A CONTRIBUTOR NUMBER THAT HAS TO GO ON THE APPLICATION FOR MORPHO-TRAK. THIS MUST BE DONE BEFORE GETTING PRINTED.

*SBI-212A Request for Criminal History Record Information

This form must NOW be submitted to the State Police electronically.  The applicant can log on to https://njportal.com/njsp/criminalrecords/ and click on the “ONLINE FORM 212A”, a highlighted block located on the lower left side of the page.  The applicant will follow the prompts for demographic and payment information.  Upon completion of the form the applicant will receive an email confirmation and receipt that will include a confirmation number.  The applicant should call the Beachwood Police Department 732-286-6000 ext. 101 and speak with Diane to obtain a nine digit “Originating Agency Identification Number (ORI) prior to completing the online 212A form. This form is required for any change in firearms application, renewals, or all handgun purchases.

STS-033 Application for Duplicate Firearms Purchaser Identification Card (FID)

  • The form is required is you need to replace a lost or stolen Firearms Identification Card
  • This form is required is you need to change your address on your Firearms Identification Card
  • This form is required if you need to replace a mutilated or unreadable Firearms Identification Card
  • This form is required if you need to change of your sex on your Firearms Identification Card
  • This form is required if you need to change your name on your Firearms Identification Card
